The link above is to a document commonly called an AOR letter or BOR letter. The terms are redundent but the reason for the letter is that aviation insurance companies will only deal with one broker that you select for your insurance requirements. This letter is used to notify the insurance companies that you have chosen N.A.I.A. to represent you in obtaining insurance coverage.
This form is used to notify N.A.I.A and your insurance company of a loss. The insurance company your coverage is placed with will have additional forms that will need to complete but this form is for initial loss information so they can set up a claim number on your behalf.
When requesting a policy to be cancelled the insurance company requires the orginal policy back or a lost policy release that verifies in writing that you wish to cancel coverage on your aviation policy.
